COPD is a serious disease, and it has been the fourth-leading cause of death worldwide. Individuals with COPD face both physical and mental challenges. Being difficult to walk due to short of breathless, being hard to do some housework, being easier to worry, sadness, leading to depression and anxiety and so on. If you or your love are suffering from the COPD, you know these better and deeper. Since the beginning, you’ll find many things in life have changed. And you have to change some habits and do more required activities to cope with it.

Quit smoking is the most important step you need to take, which smoking is the main factor that leads to the COPD. Quit smoking is very necessary even a must thing to treat the disease. If you have trouble quitting it, you can consider to consult your doctor, join a support group. Many hospitals, workplaces, and community groups offer classes to help people quit smoking. Ask your family members and friends to support you in your efforts to quit.

Also, try to avoid secondhand smoke and places with dust, fumes, or other toxic substances that you may inhale.

When having the COPD, you may have trouble eating enough because of your symptoms, such as shortness of breath and fatigue. (This issue is more common with severe disease.) As a result, you may not get all of the calories and nutrients you need, which can worsen your symptoms and raise your risk for infections. Under this situation, the small meals we ever talked will be helpful.

Then, talk with your doctor about what types of activity are safe for you. You may find it hard to be active with your symptoms. However, physical activity can strengthen the muscles that help you breathe and improve your overall wellness.

Why drug for severe COPD becomes less effective

Roflumilast, a drug recently approved in the United States to treat severe chronic obstructive pulmonary disease (COPD), increases the production of a protein that causes inflammation, which possibly results in patients developing a tolerance to the drug after repeated use and makes the drug less effective, according to researchers at Georgia State University, Kumamoto University and the University of Rochester Medical Center.

The findings, published on March 23 in theProceedings of the National Academy of Sciences USA, may help explain the development of tolerance to roflumilast and may assist with developing new therapeutics to improve the efficacy of the drug.

COPD is the fourth-leading cause of death worldwide. This progressive disease causes airflow blockage and breathing-related problems, such as coughing that produces large amounts of mucus, wheezing, shortness of breath and chest tightness. Cigarette smoking is the leading cause of COPD, according to the National Heart, Lung and Blood Institute.

Roflumilast, a new drug class for the treatment of COPD, was approved by the U.S. Food and Drug Administration in 2011 to decrease the frequency of flare-ups or worsening of symptoms from severe COPD. The drug is known commercially as Daliresp.

"There is clinical evidence showing that patients could develop a tolerance if they keep taking repeated dosing of this drug, but why or how has been unknown. If we can figure out why people have a tolerance, we can probably improve the therapeutics," said Dr. Jian-Dong Li, director of the Institute for Biomedical Sciences at Georgia State and a Georgia Research Alliance Eminent Scholar in Inflammation and Immunity.

It is important to develop new therapeutics to improve the efficacy of roflumilast because there is no effective therapeutic for severe COPD, Li said.

Roflumilast was designed to inhibit the enzymatic activity of a protein called PDE4, but the study found the drug also undergoes a complicated, unwanted process to increase the production of PDE4B2. The surge of this protein increases inflammation in ways that are dependent and independent of enzymatic activity and may contribute to the patient developing a tolerance to roflumilast. The researchers concluded the drug, even at high concentrations, cannot fully suppress inflammation because it can't suppress inflammatory response that is independent of enzymatic activity.

"We found there is some inflammatory response which cannot be suppressed by the drug because it has little to do with the enzyme activity," Li said. "The patient keeps taking the drug and over time, you give more drug and you produce more target protein, which is even more counterproductive for suppressing inflammation. Over time, we develop a tolerance."

The researchers also identified an important protein called PKA-Cβ, which can be targeted to reduce the unwanted production of PDE4B2.

In addition, they found roflumilast works with the major bacterial pathogen that causes symptoms to worsen in severe COPD to increase production of the unwanted protein PDE4B2 in a synergistic manner.

Li said the counterproductive activity of roflumilast is similar to using a fire extinguisher to bring down a fire, but at the same time, the fire extinguisher itself is increasing the amount of flammable material causing the fire

When having COPD, you’ll be firstly informed that smoking or being around secondhand smoke can make your condition worse. So stop smoking is important and keeping your home smoke-free will be helpful. But smoking is not the only COPD triggers, there are some other triggers may be lurking around your house as well. Following contents are about these common household hazards — and how to sidestep them.
1.     Avoid aerosol spray cans. Many health, cleaning, and beauty products come in aerosol cans. These may emit chemicals linked to respiratory problems. Whenever possible, opt for pump sprays or lotions instead.
2.     Watch for mold. Mold is a danger to respiratory health. Prevent mold by keeping your kitchen and bathroom well-ventilated. Seal your basement from water leaks. And make sure indoor humidity levels stay between 30% and 50%.
3.     Keep harmful chemicals out. Harmful chemicals may be found in several household items — from paint and pesticides to car exhaust. Whenever possible, reduce or eliminate them. Don’t let your car idle in the garage. Remove air fresheners, and keep your house well-ventilated
4.     Don't use a fireplace. In addition to maintaining a smoke-free house, you may need to put out the flames in your hearth, as well. Avoid using the fireplace. If you do, make sure it is properly ventilated.
5.     Clean naturally. Although keeping your house clean is important, cleaning supplies can be full of toxic chemicals. Products containing volatile organic compounds — as well as ammonia, bleach, and even some fragrances — can be harmful. Instead, clean with natural ingredients, like baking soda, or vinegar and water.
6.     Service your appliances. Home appliances can release harmful gases and other irritants. Make sure they’re inspected and cleaned regularly. This includes furnaces and central air conditioning.
7.     Avoid dusty activities. If possible, leave your house and get someone else to do chores that are extra dusty, such as vacuuming. If you must do the cleaning, reduce airborne particles as much as possible. Using a damp cloth or mop when dusting can be helpful.
8.     Be wary of carpet. If you can, remove carpeting from your home. New carpet and carpet padding can emit toxic gases. Old carpeting can collect pollutants like dust, mold spores, and pesticides. If you can’t get rid of your carpeting, be sure to keep it clean and dry. Regular vacuuming can help
9.     Monitor for carbon monoxide. If you don’t already, start monitoring your home for carbon monoxide gas — an odorless gas caused by the burning of fuels. Breathing in the gas can cause serious health problems and even death. People with respiratory problems are especially at risk. Install carbon monoxide alarms near sleeping areas.
10.   Consider cleaning the air. Some air purifiers on the market can produce air that’s 99% particle-free. Just be sure to research your options, if you choose to buy one. Not all available products are effective.

Nowadays, the higher attention that most elders pay to are the Cardiovascular and Tumour, neglecting the Lung often. Actually, Lung is the internal organs that contact the outside world directly. The refered disease as well as the chronic disease may induce the lung infection, and with high mortality rate.

The Lung infection has been the first killer to elders. It ranks third for the cause of death. Especially, When the elders have COPD, it will be much easier for them to get it like bronchitis and pneumonia(The most common infections of the lungs).
